e.g., for writing something in a PM. Or bypassing the filter in a PM. Or going OT in PM or sending someone sexual content via PM.
Does it matter if it's a to mod? Can such a ban happen? Should it happen? ETA: If it can, I have a list of people who deserve a warn, at the very least. If a mod is interested.
